Parents/Players--Please complete the 2 forms and give to your Coach/Team Manager to bring to the Registration on Thursday from 4:30-5:30 pm.
Original signatures are needed on the forms, so faxes are not acceptable.

Each player and coach/team manager will also be required to bring a photo attached to the promotion release and waiver form. This photo can be a passport id, school photo, or head shot. It will be kept on file and will not be returned.

Bring kids into the game by utilizing equipment, shorter court dimensions and modified scoring while playing in a team environment.
